Job responsibilities

Provide patient and family education related to the plan of care and discharge needs. Work 36 hours per week with 12-hour shifts. Support safe transitions of care, including handoffs and care plan updates. Assess patient status and respond to changes using established clinical protocols. Prepare patients for diagnostic tests or procedures and provide post-procedure monitoring as indicated.
